RESUMEN

Purpose: To report the long-term functional, anatomical and safety outcomes of 0.2 µg/day fluocinolone acetonide 0.19mg in patients with persistent or recurrent diabetic macular edema (DME). Methods: Retrospective, observational, single-center study of patients with recurrent or persistent DME. All patients received 0.2 µg/day of fluocinolone acetonide 0.19mg, and data were collected at baseline and months 1, 3, 6, 12, 24 and 36 after implantation. Outcomes measured included best-corrected visual acuity (BCVA), central macular thickness (CMT), intraocular pressure (IOP), and safety outcomes. Results: A total of 28 eyes from 28 patients were included. The mean age was 66.5 years (95% CI 62.8-70.2) with a mean duration of DME of 8.8 years (95% CI 7.7-10.0). Only two eyes were phakic. Mean follow-up was 25.4 months (95% CI 21.2-29.6). Mean BCVA at baseline was 48.6 ETDRS letters (95% CI 41.3-55.8) and improved as early as month 1 of follow-up with a mean gain in BCVA of 7.8 (95% CI 4.3-11.3) ETDRS letters (p<0.001). Statistically significant improvements in BCVA were also observed at months 6, 12 and 24. At baseline, patients had a mean CMT of 530.5µm (95% CI 463.0-598.0), and a decrease in CMT was observed, starting at the first month of follow-up (mean CMT reduction of -170.5µm, 95% CI -223.8- -117.1; p<0.001). Statistically significant decreases in CMT were also observed at months 6, 12, 24, and 36, with the maximum decrease observed at month 12 (p<0.001). Mean IOP at baseline was 16.4mmHg (95% CI 15.3-17.5) and nine eyes (32.1%) had an IOP ≥21mmHg during follow-up. Conclusion: Our results support the effectiveness and safety profile of fluocinolone acetonide. Although additional long-term real-world evidence is required, fluocinolone acetonide may represent a safe strategy for daily, low-dose, sustained and localized release to the posterior segment of the eye, providing both functional and anatomical benefits in DME.

RESUMO A percepção dos docentes de Ensino Fundamental sobre a sua saúde mental foi o foco desta pesquisa. Trata-se de uma pesquisa quantitativa que procura compreender as percepções das condições de saúde mental de escolas públicas no interior do Rio Grande do Sul, Brasil. O método da coleta de dados utilizado foi um questionário adaptado do Protocolo de Atenção à Saúde Mental e Trabalho respondido por 249 professores. Realizou-se a análise dos dados por intermédio do software Statistical Package for the Social Sciences - SPSS 18. Encontrou-se, como principais resultados, que índices de satisfação, motivação e identidade com a atividade docente são muito significativos. Os professores indicam comprometimento com o trabalho, valorizam a profissão, mas queixam-se de que socialmente não são valorizados, mas paradoxalmente estão muito medicados.


RESUMEN La percepción de los docentes de educación básica sobre su salud mental fue el enfoque de esta investigación. El objetivo de esta investigación cuantitativa fue comprender de esas percepciones las condiciones de salud mental de escuelas públicas en el interior de RS, Brasil. El método de la recolecta de datos utilizado fue un cuestionario adaptado del Protocolo de Atención a la Salud Mental y Labor respondido por 249 profesores. Se realizó el análisis de los datos por intermedio del software Statistical Package for the Social Sciences - SPSS 18. Se encontró, como principales resultados, que índices de satisfacción, motivación e identidad con la actividad docente son muy significativos. Los profesores indican comprometimiento con el trabajo, valoran la profesión, pero se quejan de que socialmente no son valorizados, sin embargo, paradoxalmente están muy medicados.


ABSTRACT The perception of Elementary School teachers about their mental health was the focus of this research. This is a quantitative research that seeks to understand the perceptions of mental health conditions in public schools in the interior of Rio Grande do Sul, Brazil. The data collection method used was a questionnaire adapted from the Mental Health and Work Protocol answered by 249 teachers. Data analysis was carried out using the Statistical Package for the Social Sciences - SPSS 18 software. It was found, as main results, that satisfaction, motivation and identity indices with the teaching activity are very significant. Teachers indicate commitment to work, value the profession, but complain that socially they are not valued, but paradoxically are heavily medicated.

PURPOSE: To report a case of intermediate cystinosis with an atypical presentation in which a delayed non-treatment decision has been proved right over a large period of time. METHODS: This is a unique case report of a patient who underwent full ophthalmologic evaluations including anterior chamber optical coherence tomography on a regular basis during a 7-year follow-up period. RESULTS: A 30-year-old woman with photophobia was being studied by the Department of Nephrology with a suspicion of Alport syndrome. Slit-lamp examination showed iridescent deposits throughout the corneal anterior stroma and the inferior tarsal conjunctiva bilaterally. Anterior chamber optical coherence tomography showed stromal hyperreflectivity. CTNS gene was found to be positive for c.416C>T (Ser139Phe) mutation. The patient was offered oral and topical cysteamine which was refused. After a period of 5 years of follow-up, general health status remained stable, corneal disease showed no progression and photophobia complaints diminished. However, the patient was advised to start systemic and topical cysteamine because of the unknown development of the disease. CONCLUSION: In this reported case, a delayed non-treatment decision has been proved right contrary to published evidence of active treatment of photophobia. The decision whether to treat or not to treat corneal involvement of the disease is not straightforward. Besides biomicroscopic evaluations, patients' complaints and expectations should be taken into account.

We describe a new family with a novel germline BAP1 nonsense mutation, c.723T>G, which leads to a predicted truncated protein, p.Y241*, or nonsense-mediated decay of the BAP1 mRNA. The proband had uveal melanoma (UM), and his paternal family has a remarkable history of multiple cancers. The proband's father had both pleural malignant mesothelioma (MM) and cutaneous melanoma (CM); a paternal uncle had lung cancer, CM, and UM; and a grandmother had CM. The findings in this family provide further support for the existence of a BAP1 cancer syndrome that predisposes to MM, various melanocytic neoplasms, and potentially other cancers. The fact that several members of the family manifested two or more different types of cancer suggests widespread BAP1-related tumor susceptibility targeting tissues of multiple organs. In addition, a review of BAP1 cancer syndrome families reported to date indicates that the location of the BAP1 mutation does not have any bearing on the spectrum of cancer types observed, either for mesothelial or melanocytic tumors.

OBJETIVO: Estudar por biomicroscopia ultrassônica (UBM) de alta frequência as características anatômicas da viscocanalostomia, e a relação dessas características com a redução da pressão intraocular. MÉTODOS: Estudo transversal, que incluiu nove olhos (sete pacientes) submetidos à viscocanalostomia, e posteriormente examinados por UBM de alta frequência (80 MHz). Os parâmetros da UBM avaliados após um follow-up mínimo de seis meses foram: presença de espaço intraescleral, comprimento e altura máximos do espaço intraescleral, e a espessura mínima da membrana trabéculo-descemética (MTD) residual. O sucesso cirúrgico definiu-se como pressão intraocular (PIO) <22mmHg ou redução de 20% da PIO sem medicação tópica. Possíveis associações entre as variáveis da UBM e o resultado cirúrgico foram avaliados. RESULTADOS: O tempo médio entre a cirurgia e a realização da UBM foi de 15,5 ± 8,8 meses (6 - 29 meses). Verificou-se uma redução da PIO de 23,5 ± 6,9 mmHg (13,7-32,0) pré-operatória para 14,5 ± 2,4 mmHg (10,7-17,3) pós-operatória (p<0,05). Identificou-se a presença de espaço intraescleral em todos os olhos. A média do comprimento máximo do espaço intraescleral era 1,83 ± 0,51mm; a média da altura máxima do espaço intraescleral era 0,36 ± 0,17mm; e a média da espessura mínima da MTD era 0,14 ± 0,07mm. Não foram encontradas correlações significativas entre o valor da PIO pós-operatória e o comprimento do espaço intraescleral (r²=0,359), a altura do espaço intraescleral (r²=0,017) e a espessura da MTD (r²=0,003). CONCLUSÃO: Em pacientes submetidos à viscocanalostomia, a UBM após um follow-up mínimo de seis meses identificou o espaço intraescleral em todos os olhos. Não se encontrou qualquer correlação estatisticamente significativa entre os valores de PIO pós-operatória e as características anatômicas do espaço intraescleral.


PURPOSE: To evaluate by high frequency ultrasound biomicroscopy (UBM) the anatomical characteristics of viscocanalostomy and their relationship with the intraocular pressure (IOP) lowering. METHODS: A transversal study which included 9 eyes (7 patients) undergoing viscocanalostomy and examined by high frequency UBM (80 MHz). Several UBM variables were evaluated, including the presence of the intrascleral space, the maximum length and height of the intrascleral space and the minimum thickness of residual trabeculo-Descemet membrane (TDM). Surgical success was considered to be achieved when IOP was <22mmHg or the IOP was lowered 20% or more without the use of any medication. The possible association between UBM variables and the surgical outcome was determined. RESULTS: The mean time between surgery and the UBM examination was 15.5 ± 8.8 months (range 6-29). The mean IOP decreased from a preoperative value of 23.5 ± 6.9 mmHg (range 13.7-32.0) to 14.5 ± 2.4 mmHg (range 10.7-17.3) postoperative (p<0.05). The presence of an intrascleral space was a constant finding. The mean length of the intrascleral space was 1.83 ± 0.51mm, the mean height was 0,36 ± 0,17mm; and the mean TDM thickness was 0.14 ± 0.07mm. There were a poor correlation between the level of IOP at the time of UBM and the lenght of the intrascleral space (r²=0.359), the height of the intrascleral space (r²=0.017) or the thickness of the remaining TDM (r²=0.003). CONCLUSION: In patients undergoing viscocanalostomy, UBM examination after a minimum of 6-month follow-up period showed the presence of an intrascleral space in all patients. There was no statiscally significant relationship between the level of IOP and the anatomical characteristics of the intrascleral space.

Vaccination of health care workers is an efficient way to reduce the risk of occupational infection and to prevent nosocomial transmission to vulnerable patients. Despite this, achieving high immunization rates among those professionals is a challenge. We assessed the immunization status of Residents in Pediatrics at the Federal University of São Paulo from June to December 2008. Their immunization records were checked and evaluated according to the Brazilian Immunization Schedule for health care workers. Considering all required vaccines, only 3.1% of the 64 Residents were up-to-date with their immunizations. Influenza was the vaccine with the lowest uptake (3.1%) and measles and rubella were diseases with the highest evidence of immunity (62.5% each). Only 37.5% of Residents had received three hepatitis B vaccine doses with a subsequent serology confirming seroconversion. Moreover, the vast majority of Residents in Pediatrics who were not up-to-date were unaware of the fact. Both medical schools and Pediatric Residence programs should not only offer information but also check vaccination records in an effort to keep their healthcare workers' vaccinations up-to-date.

A vacinação de profissionais da saúde representa maneira eficiente de reduzir o risco ocupacional a infecções e de prevenir a transmissão nosocomial de doenças a pacientes vulneráveis. Apesar disso, atingir altas taxas de cobertura vacinal entre estes profissionais continua sendo um desafio. Avaliamos a situação vacinal dos residentes de Pediatria da Universidade Federal de São Paulo entre junho e dezembro de 2008. Suas carteiras de vacinação foram checadas e avaliadas de acordo com a orientação do calendário nacional para profissionais da saúde. Considerando as vacinas propostas, apenas 3,1 por cento dos 64 residentes estavam em dia com sua imunização. A vacina para Influenza foi a mais negligenciada (3,1 por cento) e sarampo e rubéola, as doenças com maior evidência de imunidade (62,5 por cento). Apenas 37,5 por cento dos residentes haviam recebido três doses da vacina para hepatite B e possuíam sorologia confirmando soroconversão. Além disso, a grande maioria dos residentes que estavam com atraso vacinal desconhecia este fato. Tanto as escolas médicas quanto os programas de residência em Pediatria deveriam não apenas orientar, como também checar os registros de vacinação num esforço para manter em dia a imunização dos profissionais de saúde.

RESUMEN

Parcerias público-privadas (PPP) são arranjos contratuais onde o governo assume compromissos futuros por meio de garantias e opções. São alternativas para aumentar a eficiência do Estado por uma alocação mais eficiente de incentivos e riscos. No entanto, a determinação do nível ótimo de garantias e a própria alocação de riscos são geralmente realizadas de forma subjetiva, podendo levar o governo a ter que assumir passivos significativos. Este artigo propõe um modelo de valoração quantitativa de garantias governamentais em projetos de PPP por meio da metodologia das opções reais, e este modelo é aplicado a um projeto de concessão rodoviária. Os autores analisam o impacto de diversos níveis de garantia de receita sobre o valor e risco do projeto, bem como o valor esperado do desembolso futuro do governo em cada uma das situações, concluindo que é possível ao poder público determinar o nível ótimo de garantia em função do grau de redução de risco desejado, e que o desenho e a modelagem contratual de projetos de PPP podem se beneficiar de ferramentas quantitativas aqui apresentadas.
